Together we’ll work to reach your health goals in areas such as achieving optimal weight, reducing food cravings, increasing sleep, and maximizing energy. As we work together, you’ll develop a deeper understanding of the food and lifestyle choices that work best for you and implement lasting changes that will improve your energy, balance and health.

Filling out the forms helps Kassy determine what’s important to you and perhaps some of the trials you are having or have had in the past. The information gathered in these forms is never shared. Fill the forms out how ever you are comfortable.

Revisit Form: Once your coaching program has been set up, occasionally you may be asked to fill out the Revisit Form. This is to update your journey and allow you to see the progress you’ve made.

Women’s Health History: Specific questions regarding what you’re currently doing and what you hope to achieve. Please answer the questions as fully as possible. This initial form is a basis for what we will focus on and where we will start your journey.
